Cracked Tooth: Symptoms, Tests, and Timing

🪥 What Does a Cracked Tooth Feel Like?

Cracks can be subtle. People often report sharp pain when biting down or on release, cold sensitivity that lingers, or intermittent pain in a specific tooth. Symptoms can vary with the direction and depth of the crack, chewing habits, and existing restorations.

🧪 How We Test for a Crack

  • Bite test: A gentle, tooth‑by‑tooth test can localize pain when pressure is applied and released.
  • Transillumination under an operating microscope: Bright, focused light can highlight crack lines that are not obvious to the eye.
  • Selective staining and magnified inspection: Dyes and high‑magnification views can clarify suspicious lines.
  • Periodontal probing: Localized deep probing may suggest a more extensive crack or vertical root fracture.
  • Cold, percussion, and palpation tests: Help differentiate pulp and ligament responses.
  • Radiographs: Two‑dimensional imaging remains fundamental. CBCT may be considered when it is likely to change diagnosis or treatment planning (e.g., complex cases, non‑healing after prior care, presurgical planning). Routine use without an indication is not recommended. Note: Diagnosing vertical root fractures in previously root-treated teeth can be challenging on CBCT due to artifacts; clinical correlation is required.

⏱️ Why Timing Matters

Cracks can propagate with function. A timely evaluation helps determine whether the tooth is restorable and which option — protective restoration, endodontic therapy, or, in some cases, extraction — best fits your situation. Until you are seen, avoid chewing hard or sticky foods on the affected side.

🧭 Treatment Decisions Are Case‑Specific

  • Crazes (superficial enamel lines): Often need monitoring and protective care.
  • Fractured cusp (cracked cusp): A full‑coverage restoration may be considered; endodontic therapy is recommended if the pulp is symptomatic or involved.
  • Crack extending into the pulp: When restorable, root canal treatment followed by a well‑sealed full‑coverage restoration may help maintain function.
  • Split tooth or vertical root fracture: Frequently non‑restorable; careful evaluation is performed to confirm the diagnosis and discuss options.

🔬 How Microendodontics Helps

The operating microscope improves visualization of crack paths and tiny canal anatomy. When indicated, CBCT with a small field of view can aid in assessing lesion extent or surgical planning. These tools complement — not replace — clinical judgment.
